Breakfree & Assess is a leading concierge program for adults seeking a unique, collaborative 360° assessment with a personalized team of superior providers. The program helps clients through individualized, tailored treatment and experiences, improving quality of life through a deeper understanding of needs and self-care.

Each Breakfree & Assess team is composed of experts working with an individual to create a clear, custom multi-disciplinary set of findings. Our embedded team of professionals assists in motivating change while reducing anxiety to allow for more accurate findings.

The 21-day experience is for adults who:

Exhibit persistent treatment-resistant mental health issues
Lack an accurate diagnosis with cause and condition
Have a history of instability resulting from crisis or impairment, mood disorders, dual diagnosis and/or substance use

Insights for Action: Comprehensive Findings Report. Each client leaves with a Comprehensive Findings Report, a diagnosis set with clinical summary, long-form report, and an aftercare plan.

The 360° Assessment

During each 360° assessment, clients step through eight core processes with 24/7 awake team support. Diagnostic assessments, inventories and testing may include one or more of the following.
